St. Catharine CCD Registration

Please fill in the information below for each student you are registering.  The top section for each child must be filled in for all students.  If the student is NEW to the St. Catharine Religious Ed program, please fill in the requested information regarding other sacraments as specified below:

• Use their Baptized Name.
• If they received any previous sacraments at St. Catharine, please provide the date.
• If they received any previous sacraments at another parish, please provide us with a copy of the certificate for this sacrament (Baptism or First Communion)
• If you are transferring from a Religious Education program at another parish, that information is required.  Please request the previous parish to notify us by letter and include any sacraments your child/children have received.  They may mail this information to the St. Catharine Religious Education Office at 180 Rodney Street, Glen Rock, NJ  07452.
• Please be sure to fill out the full name and address of previous Churches as we mail the parish the dates of your child’s sacraments for their permanent records.
